What We Offer

Embrace Hospice offers compassionate care, and expertise to our patients, and families. We utilize a team approach to care for their physical, spiritual, emotional, and social needs.

LEVELS OF HOSPICE CARE

Routine Home Care

Hospice team members visit the patient through scheduled weekly visits. The amount of visits is determined by each individual patient’s plan of care.

General Inpatient

For pain and symptom management that cannot be provided at home. It is provided in a contracted facility (hospital or Embrace Hospice House).

Respite Care

Short-term care is used only to relieve the family/person caring for the patient at home. It is a 5-night benefit provided in a contracted resident or long-term care facility meeting certain criteria.

Considering hospice comes with many questions, uncertainty, anxiety, and even fear. We are here to help. When you choose Embrace Hospice, you’ll receive the education, resources, and support you need, right from the start.

My loved one may be ready for hospice.
What Now?

A hospice referral from your loved one’s primary doctor can be accepted but is not required. Anyone, including a family member, may make a hospice referral by calling Embrace Hospice at 843-234-8781.

Our professionals will meet with you and your loved one for an in-person assessment. We’ll gather information such as recent health declines and concerning symptoms. We’ll also educate you and your loved one about the hospice benefit.
