Can Hemorrhoids Cause a Positive Cologuard Test?
Hemorrhoids—those pesky, swollen veins that can cause discomfort and itching in your nether regions—are more common than you might think. While they can be bothersome, they’re generally not something to panic about. However, if you’re using a Cologuard test to screen for colorectal cancer, you might be wondering if those annoying hemorrhoids could affect your results. To clear things up, we reached out to Dr. Vivek Choksi, a top gastroenterologist at CMC Digestive Health, to help us understand how hemorrhoids might influence a Cologuard test and why a colonoscopy is still considered the gold standard for colorectal cancer screening.
Understanding Cologuard and Hemorrhoids
Cologuard is a non-invasive screening test that analyzes stool samples for signs of colorectal cancer and precancerous polyps. It looks for DNA markers and occult blood, which can indicate the presence of cancerous or precancerous cells in the colon. However, certain conditions, including hemorrhoids, can sometimes lead to a positive result.
Dr. Choksi explains, “Hemorrhoids, particularly if they are bleeding, can contribute to the presence of blood in the stool. Since Cologuard tests for occult blood, this can potentially result in a false-positive test. It’s important to remember that while hemorrhoids are a common condition, they are not related to colon cancer. However, their presence can complicate the results of a Cologuard test.”
Importantly, if your Cologuard test comes back positive, it’s essential to follow up with a colonoscopy. A positive result doesn’t necessarily mean you have cancer, but a colonoscopy is needed to confirm and further investigate any abnormalities.
Why Colonoscopy Remains the Gold Standard
While Cologuard offers a convenient and non-invasive method for colorectal cancer screening, it has limitations. Dr. Choksi emphasizes, “Colonoscopy is considered the gold standard for colorectal cancer detection for several reasons.”
- Direct Visualization: Colonoscopy allows for direct visualization of the colon and rectum. This means that any abnormal growths, polyps, or signs of cancer can be identified and biopsied on the spot. In contrast, Cologuard can only detect potential issues through stool samples and cannot provide a direct view of the colon.
- Higher Sensitivity and Specificity: Colonoscopy has a higher sensitivity and specificity compared to stool-based tests like Cologuard. It can detect even small polyps and early-stage cancers that other tests might miss. Dr. Choksi adds, “Colonoscopy is more accurate in diagnosing colorectal cancer because it allows for a comprehensive examination of the entire colon and rectum.”
- Polyp Removal: During a colonoscopy, any polyps found can be immediately removed and analyzed. This is crucial because certain types of polyps can develop into cancer over time. “The ability to remove and analyze polyps during the procedure significantly reduces the risk of developing colorectal cancer,” notes Dr. Choksi.
- Follow-Up and Monitoring: Colonoscopy also allows for ongoing monitoring and follow-up. If any abnormalities are detected, additional procedures or treatments can be planned based on the findings. This continuous monitoring is essential for maintaining colon health.

So remember, while hemorrhoids can potentially lead to a positive Cologuard test, they do not necessarily indicate the presence of colon cancer. As Dr. Choksi highlighted, colonoscopy remains the gold standard for colorectal cancer detection due to its ability to provide direct visualization, higher accuracy, and the opportunity for immediate intervention. If you have concerns about your colorectal health or screening options, consult with a healthcare professional to determine the best approach for your needs.
