Dear Provider,

Thank you for your interest in referring your patient to Conway Medical Center Infusion Services! For your convenience, we have many prebuilt treatment plans listed below. If you are unable to locate a Treatment Plan for the medication you wish to order, you may use the “Requested Treatment-Ambulatory Infusion” plan below or email the Director of Infusion Services (tyler.clay@cmc-sc.com) to request a treatment plan be built.  Treatment Plans are provided in a fillable PDF format or may be printed and filled out manually. Please note all treatment plans require an original signature at the bottom of every page. Once plans have been completed, please fax them to 843-234-5460.

 Once completed treatment plans are received, our Medication Management Specialist will begin the process of obtaining Prior Authorization for your patient. Once authorization has been received, our Medication Management Specialist will work with central scheduling to get your patient scheduled in a timely manner. Please note: Infusion Services processes referrals and authorizations five days a week (Monday through Friday), and generally has a 24 to 48-hour turnaround time, however, some insurance companies reserve the right to allow for 14 days for their own internal review of authorization requests.
